Greater Lynn Senior Services provides a wide range of social services for elders and people with disabilities -- information, transportation, meals, advocacy, homecare, housing, and more--for one reason: to help those 60 and older in Lynn, Lynnfield, Nahant, Saugus and Swampscott live fuller, more independent lives, safely and with dignity.
40 hours per week.
The Case Manager has daily responsibility for developing, authorizing, coordinating, and monitoring community-based services to an assigned number of frail elders. The process begins with a comprehensive needs assessment, completed as part of an interdisciplinary team. Requires a thorough knowledge of available community services, an understanding of both formal and informal support systems, and an ability to interface and collaborate with those systems to ensure the elder's needs are met. The position entails extensive coordination, follow-up, and feedback to clients and services providers, as well as record-keeping in compliance with regulations.
B.A. degree in Human Services or related field with 1-2 years experience in Human Service. Must have a driver's license and reliable transportation. Excellent communication and documentation skills. Proficiency with Microsoft office, proficient computer and administrative skills required. Ability to make proper judgments, demonstrated leadership qualities, possesses good written and verbal skills, and maintains effective interpersonal relationships with staff members and consumers. Must be LSW eligible.
