The Home Care Case Manager has the responsibility for developing, authorizing, coordinating, and monitoring community-based services to older adults and people with disabilities. The process begins with a comprehensive needs assessment, completed as part of an interdisciplinary team. Requires an ability to meaningfully engage potential consumers and collaterals in the community, a thorough knowledge of available community services, an understanding of both formal and informal support systems, and an ability to interface and collaborate with those systems to ensure the consumers' needs are met. The position entails extensive coordination, follow-up, and feedback to consumers and service providers, as well as record keeping in compliance with regulations.



Bachelors degree in Human Services or related field. One to two (1-2) years' experience in Human Service preferred. Ability to actively participate in multi-disciplinary team environment with excellent written and verbal communication and documentation skills required. Ability to make proper judgments, demonstrate leadership qualities, and maintain effective interpersonal relationships with staff members and consumers. Bilingual (Khmer or Russian). Must have a driver's license and reliable transportation.



Full time - 40 hours per week.
