Full time - 40 hours per week.


The Healthy Hearts RN/Prevention Wellness Trust Fund (PWTF) and Referral Liaison will work collaboratively with Healthy Hearts Team Leader and GLSS PWTF Embedded Social worker at Lynn Community Health Center (LCHC) to maintain current protocols and e-referral mechanisms. He/she will work in partnership with GLSS Staff and LCHC Teams to facilitate ongoing communication and exchange of critical information.

Must have a current Massachusetts RN License, a minimum of five years of clinical experience and two years of experience in community nursing. Must demonstrate leadership skills and have the ability to work independently and within a multidisciplinary team under the supervision and guidance of the Healthy Hearts Initiative Team Leader. Candidate must have reliable transportation and a valid driver's license to make visits. Must provide support for Community Health Care Workers. Maintain effective organizational, communication, and interpersonal skills. Demonstrate an understanding and commitment to the Healthy Hearts Initiative (HHI). Computer literate with proficiency in Microsoft Office, including: Word, Excel, and Outlook. Ability to learn and master industry specific software and programs. Fluency in Spanish, or any other language, preferred.
