The LongTerm Services Support Coordinators, (LTSSC), are a Greater Lynn Senior Service (GLSS) employee leased to The Greater North Shore Link, Inc. which is a strongly woven consortium as well as an over-arching non-profit organization serving nineteen municipalities in southern Essex County. Working with a multitude of diverse partnering agencies across both health and social service disciplines the Link core leadership is comprised of the Independent Living Center of North Shore and Cape Ann, Inc., Greater Lynn Senior Services, Inc. North Shore Elder Services, Inc., SeniorCare, Inc., Elder Service Plan of the North Shore, Inc. and North Shore Career Center. The Link is dedicated to promoting consumer choice and self- management as well as greater community inclusion, access, integration, mobility and robust livability.


Minimum of a Bachelor's degree in Social Work, Counseling and/or Human Services field from an accredited institution. A minimum of five (5) years' experience providing care management in a health and/or human services organization, specifically to anyone twenty-one (21) years and older participating in the One Care Program.  Experience working with diverse population and individuals living with disabilities preferred. Ability to attend and complete required trainings. Excellent computer skills required. Exceptional interpersonal and assessment skills are required in order to make well informed decisions. Applicants must reliable transportation and a valid driver's license since fifty (50%) percent of this position is out in the community. Bilingual Spanish speaking a plus.


Full Time, 40 hours per week (Monday-Friday)
