Duties:

The Intake Case Manager has the responsibility for developing, authorizing, coordinating, and implementing community-based services to older adults and people with disabilities. The process begins with a comprehensive person-centered needs assessment, completed as part of an interdisciplinary team. Requires an ability to meaningfully engage potential consumers and collaterals in the community, a thorough knowledge of available community services, an understanding of both formal and informal support systems, and an ability to interface and collaborate with those systems to ensure the consumers' needs are met. Requires a commitment to consumer-directed service models and experience with person-centered care. The position entails extensive coordination, follow-up, and feedback to consumers and service providers, as well as record keeping in compliance with regulations. As a member of the interdisciplinary Home Care management team, the Intake CM actively collaborates with the team to ensure the provision of person centered services to older adults, people with disabilities, and their caregivers according to the Mission, Vision, and values of GLSS.

Qualifications:

Minimum of a Bachelor's degree in Human Services or 50% of Bachelor's Degree completed with degree obtained within three (3) years of employment. One (1) to two (2) years' experience in Human Service. Must demonstrate a passion for supporting consumers in achieving their personal goals related to wellness and community living. Excellent communication, organization, time management, and documentation skills. Proficiency with Microsoft office, proficient computer and administrative skills required. Ability to make appropriate judgments, possesses good written and verbal communication skills, and able to maintain effective interpersonal relationships with staff members and consumers. Must have a driver's license, clean driving record, and reliable transportation. Bilingual - Spanish speaking is a must!

Hours:

Full Time, 40 hours per week (Monday-Friday)
